Application in Form I under the Rules in duplicate to the Village office where the applicant has been residing continuously for not less than one year on the date of application accompanied by a Medical certificate in form X under the rule from Medical Officer concerned -
Who can apply?
Destitute leprosy patients who are residing within the Kerala State for a continuous period of not less than two years immediately preceding the date of application. A destitute Leprosy Patient - €¢ A person whose monthly income from all sources is below Rs. 200/- per mensem and who has no relatives viz. husband, wife, son, father, mother and above 21 years of age
€¢ Provided a person is deemed to be a destitute of none of the relatives has a monthly income of above Rs. 200/- per mensems
€¢ Provided the relatives of the categories of the categories mentioned above are continuously missing for more than seven years, the applicant shall be deemed to be one having no relative
€¢ A son adopted under proper authority will be deemed to be a son and a stepson will not be deemed to be son under the rules

Approval Procedure
i. The Tahsildar will sanction assistance for the record on production of certificate, if the patient appear before the government doctor not below the rank of an Assistant Surgeon in charge of P>H. Centers, Government Rural Dispensaries, Government dispensaries or Hospitals having jurisdiction over the place of residence of the patient within one month after expiry of the original assistance and who on medical examination certify that his state of health in such that further assistance has to be given and that he has not secured admission to any leprosy hospital or sanatorium and that he is regularly undergoing treatment in the SET Centre according to the prescription from time to time.
ii. The assistance shall not be continued beyond two years in any case.
iii. The application for assistance received after three years from the date of expiry of the original assistance will be treated as fresh under the scheme.
The verification and sanction in respect of all applications shall be completed within two months of receipt of the application by Village Officer. In case of delay the sanction shall have effect from the first of the third month of receipt of application and in such cases, arrears will be paid in lump -
