How To Apply  Prescribed form is available for Certificate            
for Determining family income-:

Whom to apply To concerned Village Officer. For special cases, the Certificate is from Tahasildar, application is given to the Taluk Tahasildar

Documents required . Ration Card
. Salary Certificate, if employed
. Pension payment order
. In case of   NRI, an affidavit  revealing the income details.  In this case, the affidavit is given by applicant and in the case of students the affidavit is given by parents/guardian
. Income from landed property
Fees A court fee stamp worth of Rs. 5/- to be affixed in the application form.  The applicants who belong to SC/ST is  exempted from affixing Court Fee Stamp

In the case of Salary Income, DA and HRA is not considered for calculating Family Income for fee concession to the students in Arts and Science Colleges. 

DA & HRA is not included for calculating income for seat reservation in Graduate/Post Graduate Courses in Provisional Colleges to the students who belongs to Economically and Socially Back Ward Class on the basis of Kumara Pilla Commission.  But for fee consession for this category, DA& HRA has to be included for calculating income
