for Filing Complaint
Note
1.
The Form can be copied out on plain paper by the Complaint.
Additional pages can be added according to the requirements
of the nature of description of the alleged corruption (Col.
No: 5)
2.
Every Complaint and the documents accompanying it shall
be in quadruplicate, accompanied by as many number of spare
copies as there are public servants against whom allegations
of corruption have been made in the complaint.
3.
The affidavit accompanying the complaint may be sworn or
affirmed before any Judicial Officer, A District Registrar
or Sub-Registrar, the Chief Ministerial Officer of any Civil
or Criminal Court in the State of Kerala, A member of Parliament
or of the Legislature of any State in India, the Chairman
or Executive Authority or a member of any Municipal Council,
Corporation or other local authority in India, a Gazetted
Officer in the service of the Central Government or the
Government of any State in India, a Commissioned Officer
in the Defence Forces of India or a Pleader.
4.
The person whom an affidavit is sworn or affirmed shall
state the date in which, and the place where the same is
made, and sign with his name and designation and affix his
seal, if any.
5.
No fees prescribed for filing complaint.
6.
Party can present their case in person or through pleaders.
7.
When a complaint is admitted, stamped covers with acknowledgment
due has to be produced in person or through post for issuing
notice to the respondents.
